Job Summary:

• Support continued development and implementation of a Discharge Decision Algorithm Tool and Informed Choice Script
  
• The Director will serve as an advisor to Hospital Medicine Department, Hospitalists, ED Providers, and Community Physicians to ensure alignment and support of Discharge Decision tools, programs, and processes
  
• Link into continuous review processes to observe processes/tools in action and identify improvement ideas for Discharge Decision tools, programs, and processes
  
• Provide Inpatient Case Management expertise and guidance, focused on Best Practice Multi-Disciplinary Rounding, Discharge Planning, Documentation
• Serves as the Hospital Patient Throughput Subject Matter Expert
• Physician-specific Performance related to Skilled Nursing Facility (SNF) and Preferred Network Utilization
  

Minimum Qualifications:

• Bachelor's degree in healthcare related field
  
• 5 years of operational or clinical leadership/management in Inpatient Care Management or Discharge Planning services
  
• Registered Nurse licensed to practice in Minnesota
  
• Demonstrated history of developing and implementing data-based performance improvement strategies
