Summary:
Responds to the health education needs of the department; develops and implements community and professional educational program plans, intervention strategies, educational trainings, meetings and seminars; assists in the collection of data and resources for development and evaluation of grants, research, educational programs, educational or promotional materials in the department and through outreach programs; works under the supervision of the Community Outreach Manager. The Health Education Specialist will provide virtual navigation support for Federally Qualified Health Centers (FQHCs) and/or Indian Health Service (IHS) clinics in the Central NY region through targeted and culturally tailored education and navigation, primarily serving the rural and tribal communities with a special focus on breast and prostate cancer screening and management of cooccurring conditions (e.g. diabetes, obesity). Additionally, will offer support and assistance through the screening and any potential diagnostics and treatment as needed; navigates and tracks patients through health care systems, including primary care offices, hospitals and outpatient clinics, insurance and payment systems, patient-support organizations, cancer services programs and other components of the health care system. Will work to provide remote services at the FQHCs and/or IHS clinics in conjunction with primary care providers (PCPs) to coordinate cancer screening, outreach programs, local and regional health centers and transportation services to facilitate care; additionally support telephonic navigation and call center functions for Cancer Screening and Center for Indigenous Cancer Research team. Serves a s a resource for cancer screening services and management of co-occurring conditions along with PCPs and clinic staff. Uses Electronic Medical Record (EMR) systems to identify and flag patients due for cancer screenings for targeted education and navigation. Work closely with Community Outreach Manager and key stakeholders at clinics to define and execute a workflow meeting the needs of the community and practice.

Qualifications:
Required Education and Experience
1. Master's degree in public health, health education or a related field and the equivalent of one (1) year of full-time experience or a combined one (1) year of full-time experience and internship experience in a community, patient or customer-oriented support; or
2. Bachelor's degree in public health, health education or a related field and the equivalent of three (3) years of full-time experience or a combined three (3) years of full-time experience and internship experience in a community patient or customer-oriented support; or
3. Associates degree in public health, health education or a related field and the equivalent of five (5) years of experience in a community patient or customer-oriented support; or
4. High School diploma or High School Equivalency diploma and the equivalent of seven (7) years of experience in a community patient or customer-oriented support.
NOTE:
Required degrees must have been granted by an accredited college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.

Preferred Qualifications

The preferred candidate should have experience with health education and/or eligible for health education specialist certificate and experience with community health centers and physicians in our region. In addition, the preferred candidate should have knowledge of cancer screening, cancer prevention programming, and patient navigation, including virtual and telephonic.
