Summary:

Responsible for providing individual, family, group therapy and case management services to youth and families.

Responsibilities:

Responsible for all pre/post data collection and grant required outcomes per youth and families on case load.

Conduct intakes and assessments; provide individual, family and/or group therapy, provide case management per client need; utilizes evidenced based curriculum techniques as required by grant.

Responsible for transportation coordination for special activities and events.

Responsible for communicating with referral sources on youth/families progress and outcomes.

Responsible for completing all daily, weekly and monthly progress notes, treatment plans, treatment plan reviews, progress updates and discharge plans for each youth and family on case load.

Additional Job Information:

Required Work Experience: No prior work experience required. Other Information: PALS certification will be accepted, for BLS if new hire works with children. New hires will be granted a 6 month period, from their date of hire, to obtain certification.Additional Education Info: Masters degree in Social Work, Marriage and Family Therapy, Psychology, Mental Health Counseling, or related therapy field.
