Minimum Qualifications: - Bachelor's degree in social services or a related field, associate's degree with 2 years' experience or 4 years of related experience.

Summary: The case manager is responsible for providing comprehensive case management services to clients including but not limited to intake assessments, benefit assessments, goal setting, case plan development, home visits, progress monitoring via standardized testing tools, client advocacy and referrals while maintaining compliance with established state and federal standards and requirements.

Essential Functions:
1. Conduct home visits to assess client's needs for case plan development, establish rapport, and obtain standardized assessments.
2. Make appropriate referrals based on client case histories and needs.
3. Assist clients in completing the Medicaid/CHIP application process as well as any other benefits in which they are eligible.
4. Provide health education to clients.
5. Assist in conducting outreach duties as assigned by program director.
6. Attend trainings as assigned by program director.
7. Comply with safety procedures in accordance to program requirements.
8. Meet all deadlines by program director and federal partners.

Measurable Deliverables:
1. Maintain a client caseload of 40-60.
2. Meet documentation deadlines in accordance to program requirements.
3. Conduct routine home visits following risk assessment requirements.
4. Maintains client contact following risk assessment requirements.
5. Obtain and enroll in continuing education/trainings required for recertification(s).
6. Enter information in Salesforce as required by agency policy, procedures, and guidelines.
7. Increase client's knowledge and positive changes in attitude as measured by standardized tests.
8. Participate in scheduled meetings and team decisions and operations.
9. Participate in workshops, seminars, education programs and activities that promote professional growth and development.
10. Compile, prepare, submit and maintain accurate records, files, forms, and additional information in accordance with agency policies, licensing and/or contract requirements.
11. Implement BCFS safety protocols at all times.
12. Maintain BCFS HHS professional and ethical standards of conduct outlined in BCFS employee handbook including demonstrating respect for agency staff, children, and community members and complying with required dress code at all times.

Credentials, Skills and Abilities:
1. Maintain current licenses including any certificates required for the position including, but not limited to: Driver's License, First Aid CPR AED, Application Assistance Navigator, Community Health Worker, Mental Health First Aid, and Child Passenger Safety Technician.
2. Meet all federal and state regulatory guidelines and standards applicable to this position
3. Pass a pre-employment drug screen and random drug screens throughout employment.
4. Provide proof of work eligibility status upon request.
5. Pass a pre-employment and biennial criminal background checks.
6. Drive a vehicle and comply with safety procedures in accordance to program requirements
7. Demonstrate the ability to:
• Respond sensitively and competently to the service population's cultural and socio-economic characteristics
• Work in a fast paced environment and maintain emotional control and professional composure at all times.
• Communicate effectively in writing and verbally in English (Spanish highly preferred)
• Organize and prioritize responsibilities and duties efficiently
• Provide crisis intervention according to training provided by BCFS HHS when needed to maintain a safe environment
• Maintain computer literacy required to meet the responsibilities of the position
• Maintain confidentiality in all areas of the service population and program operations.
