Global Communities is seeking a Perinatal Navigator to join the Healthy Start program team in San Diego, California.

Healthy Start is a national initiative to improve perinatal family health and works with individuals and partners to reduce disparities in San Diego County. Ideal candidates have experience with childbirth education, doula work, lactation and/or parenting education. Perinatal Navigators work under direction of the Healthy Start Project Director and will work primarily with families in the field (in San Diego), with additional work from the San Diego Murphy Canyon office. This position is eligible to work a hybrid remote work arrangement.

The Perinatal Navigator independently manages a caseload of Healthy Start program participants, including providing culturally relevant and participant-centered services during pregnancy and early childhood with a focus on diverse communities, including African American and Black Immigrant families. The Perinatal Navigator conducts outreach, enrolls participants, and provides health education related to pregnancy, childbirth, and infant care, primarily through one-on-one visits with families in their home or other locations, including virtually during COVID restrictions. In addition, this role interacts with other program staff and external partner agencies to exchange information to ensure optimal health outcomes for clients while ensuring HIPAA compliance.

There is flexibility to work full-time 40 hours per week or part-time 20 hours per week.

COMPENSATION

$48,640 - $60,800 annually

Responsibilities

Responsibility Area: Outreach, Enrollment, & Networking

  • Conduct outreach, enroll participants, and provide case management to 40+ pregnant individuals and their families each year, with an emphasis on underrepresented communities.
  • Serve as a positive representative for the program by participating in collaborative meetings with community partners.


Responsibility Area: Perinatal Health Education
  • Provide educational, culturally relevant and language appropriate visits to program participants relating to healthy pregnancies, childbirth, breastfeeding, and infant care.
  • Empower individuals and families by building resilience and reducing stress during pregnancy through promoting health behaviors and services that reduce prematurity, infant/maternal mortality and low birth weight outcomes.
  • Identify participants in need of additional services (medical, social and economic and mental health), make appropriate referrals to community agencies and document follow-up contact.


Responsibility Area: Data Collection
  • Collect and enter relevant project data in Salesforce to document each families' strengths, needs, and outcomes into a project database.
  • Conduct cleaning of data as needed in collaboration with the Project Director to ensure accurate reporting.
  • Ensure HIPAA compliance for all project related data.

Other
  • Support project organized community events, health education and support group activities.
  • Participate in agency, project, and partner activities and meetings.
  • Maintain continuing education as appropriate. Perinatal Navigators will be supported to achieve certification in childbirth, parent, and lactation education.
  • Perform other duties as assigned.


POSITION SPECIAL RESPONSIBILITIES:
  • Must be able to work occasional evenings and one Saturday each month.
  • Must be able to work additional during peak periods.
  • Must have reliable transportation for regular travel around San Diego County.

Knowledge, Skills and Abilities
  • Socio-cultural experience with the populations served, along with knowledge of and respect for the values and beliefs of African American and/or Black Immigrant women and communities.
  • Knowledge of women's health, including prenatal and post-partum concerns.
  • Experience in topics of mental health, childbirth, infant behavior and development, and local community and social service resources.
  • Proficiency in computers and use of MS Word and Excel required.
  • Able to work both independently and in a team.
  • Detail-oriented with strong communication, presentation, and interpersonal skills.


Qualifications
  • Specialized knowledge in work discipline and four years of relevant work experience, including health, psychology, child development or social work.
  • Specialized training in childbirth, doula, postpartum doula, or lactation education is a plus; at least two of these trainings will be provided within the first year of employment.
  • Fluent in English (read, write, speak) is required, second languages are a plus (Somali, Swahili, Haitian Creole, and Spanish).
  • Experience in the following areas is a plus: home visiting, case management, Community Health Worker, benefits eligibility including Covered California.
  • A passion for the mission and values of Global Communities.
  • Must have U.S. work authorization.
  • Must be available for occasional travel within California and US.
