JOB SUMMARY

  • provide direct support to people with disabilities for routine and as needed health care needs, conduct various assessments, address minor illnesses/injuries and ensure proper follow-up,
  • coordinate care with and make referrals to other multi-disciplinary health care providers/facilities as appropriate,
  • serve as a member of the Interdisciplinary Team and participate in the ISP planning and implementation processes as required per regulations,
  • ensure adequate nursing personnel is scheduled at all times for the homes they oversee,
  • provide supervisory oversight to LPNs as necessary,
  • responsible for ensuring that assigned nursing personnel timekeeping and related functions are completed in accurate and timely fashion (as needed); and,
  • ensure safety of medical care performed during the day program hours
  • Assess person served on every shift and report any changes in health status immediately
  • Understand the basic fundamentals of nursing
  • Understand or be willing to successfully complete all competencies with VOA and DDS on IDD services and the person served within 45 days of hire.
  • Guide and supervise the Direct Support Professionals in ensuring medical safety is maintained with person served.
  • Advocate for the person served and coordinate with other multidisciplinary team members regarding optimal treatment for person served.
  • provide general DC programs support when requested (per regulations)
  • Remain compliant with regulations of regulatory bodies(CARF, DDS, DC Board of Nursing, VOA, etc.)within the Scope of his/her highest Nursing Practice.

REQUIREMENTS:
  • Be a graduate of an accredited school of nursing with registration and license to practice as a Registered Nurse (RN) in the District of Columbia; and two years RN experience.
  • Be able to read, write, and speak the English language and have the ability to communicate effectively with the people to whom services are provided and all stakeholders.
  • Ability to document all healthcare and medical needs as required by all regulated licensing and monitoring entities.
  • Ability to read and interpret, and document clinical paperwork such as safety rules, medical orders, admission and discharge instructions, operating and maintenance instructions, and policy/ procedure manuals.
  • Ability to write routine reports and correspondence in a timely manner.
  • Ability to perform basic computer and office equipment skills (EMR and Word).
  • Ability to utilize the RN scope of practice regulations for the District of Columbia to present accurate health care information in one-to-one and small group situations with people supported, families, guardians, and other medical professionals within the interdisciplinary team.
  • Attend all meetings scheduled by the Director of Nursing Services and the IDD Program Manager (As needed)
  • Provide TME training to DSP staff if one is a DDA Qualified TME trainer with the DC DOH(plan to become TME Trainer Certified)

New Employees must successfully complete:
  • All Volunteers of America Chesapeake orientation components within 45 days of hire.
  • All requirements as defined by the DC DDA and other monitoring entities of Volunteers of America Chesapeake-DC.

Note: All employees must maintain all on-going training, certifications, and be otherwise compliant and in good standing with all program requirements as a condition of continued employment. Additionally, all RNs will keep their registrations and license(s), and CPR card in good standing at all times.

PRINCIPLE ACTIVITIES:

Clinical
  • Be available, on-call, for nursing consultation and/or treatment of minor illnesses or injuries and render nursing assistance in medical emergencies remaining within the RN scope of practice.
  • Develop rapport with each person receiving nursing care. Demonstrate respectful interactions with high-quality customer service skills at all times.
  • Participate and lead in the safe and initial admission process, evaluation and treatment plan designed for each person admitted to this program within a safe and regulated manners.

  • Participate as a member of the Interdisciplinary Team providing general health counseling, review of medical procedures, medication and medical options with the person, their family, guardian and staff.
  • Complete the following specific on-going tasks.
    • Make appropriate entries in nursing notes related to the care of the people supported.
    • Audit all nursing medical books routinely as designated.
    • Monitor adaptive equipment and update Health Passport accordingly.
    • Collaborate with other clinical staff to discuss necessary changes that may be needed in home per the Individual Treatment Plan to optimize the general medical status of people supported.
    • Maintain all appropriate medical documentation such as medical consultations, medical history, medication errors, etc. (Report any errors or issues immediately per regulations)
    • Help coordinate all hospital discharge instructions for person served (if needed).
    • Ensure successful completion of yearly nursing skills test with Director of Nursing.
    • Prepare medical summaries and attend ISP meetings as scheduled.
    • Complete identified assessments as needed at least annually for each person supported.
    • Communicate with the RN Supervisors regarding each persons medical needs including routine, and well as, emergency treatments.

Management
  • Ensure that LPNs are providing coverage to IDD programs and supporting the DSP in delivery of services (if LPN is available on shift).
  • Provide and ensure that LPNs are aiding person served and family to coordinate all medical appointments in a timely manner.
  • Develop and maintain Health Care Management Plans for all individuals while in the Day Program and ensure that appropriate staff are involved in the process. Ensure up-to-date documentation of all follow-up consults, such as, Health Passport, Nursing Assessment, Glasgow Scale, AIMS, Fall Risk Assessment, Braden Scale, Vaccinations, Annual Health Form, Self-Administration of Medication Assessment.
  • Total of Person Served on a daily basis ranges from 10 to 20 persons served(max capacity).

Administrative
  • Provide appropriate in-service training to staff in basic health care and hygiene skills.
  • Actively participate in activities related to medication administration:
    • Perform periodic medication administration review(weekly and as needed). Identify any errors and report accordingly.
    • Train, track LPNs in Medication Administration training and all required documentation including the proper completion of supervisory medication passes( if needed).
    • Train and monitor TME trainings and document appropriately (if needed).
    • Regularly monitor the Medication Administration Record (MARs)
    • Regularly monitor and appropriately document the storage, efficiency, order updates, side effects, safety, etc. of medications in your care.
  • The RN should have working knowledge of community resources and a plan for accessing those resources.
  • RN will collaborate with the other IDD RN Supervisors regularly for transparency and to ensure that optimal treatment is given to the person served.
  • The RN must adhere to HIPAA regulated guidelines.
  • Help coordinate all nursing-related information during program transfers in case of an emergency situation. The nurse will also perform transfer teaching and training to staff, and update the Health Passport.
  • Maintain up-to-date knowledge of Volunteers of America Chesapeakes and Carolinas policies and procedures.
  • Be knowledgeable of the rules and regulations of the relevant service environments including ICFs, Waiver, Medical Respite, and Res Hab and adhere to the program guiding principles.
    • Contribute meaningfully during internal audits and external monitoring visits including: Evans Class Monitors, Quality Trust, HRLA, DOH, PCR, DDS, DDA in the District of Columbia.
    • Complete all necessary requirements related to MCIS-generated issues within the designated timeframes.

PHYSICAL REQUIREMENTS:

  • The ability to safely operate a motor vehicle to transport oneself, people and program supplies as necessary.

  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.
  • The physical ability to move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
  • The physical ability to lift, transfer, carry, push, pull a person served when required through routine assistance or during an emergency.
